The Intake Process

Below you will find a form for inquiry. The form takes less than a minute to complete.

Once this is submitted, I will receive an email with the information you've given me. I will respond as quickly as I can to schedule your free consultation. 

During our consultation, we will discuss what you'd like to achieve in therapy, your comfortability with Telehealth services and to make sure this is a good fit for you. 

Lastly I will collect your insurance information and have that submitted to your providers so we can get started! Once your insurance is approved we can begin sessions rather quickly.
